Нарушения менструального цикла (НМЦ) широко распространены среди девочек подросткового возраста.Цель. Изучение уровня кисспептина плазмы крови у девочек-подростков с ожирением и НМЦ.Материалы и методы. В исследование включены 80 девочек-подростков с ожирением. 1 группа — с НМЦ, 2 группа — без НМЦ. Включение в группы через 1 год и более после менархе. Лабораторное обследование включало оценку углеводного и липидного обмена, гормональное обследование.Результаты. Достоверных различий по SDS ИМТ между группами не выявлено (р = 0,486 и р = 0,459). Достоверно выше в 1 группе: нарушение толерантности к углеводам (НТУ), гипертриглицеридемия (р = 0,022), уровни кисспептина, лютеинизирующего гормона (ЛГ), общего тестостерона, антимюллерова гормона (АМГ), индекса свободных андрогенов (ИСА) (р = 0,001, р = 0,008 р = 0,026, р = 0,014, р = 0,027 соответственно). Уровень эстрадиола в группе 2 был достоверно выше, чем в группе 1 (р = 0,012). По результатам ROC-анализа установлено оптимальное значение уровня кисспептина плазмы крови, равное 53,56 пг/мл.Заключение. Пациентки с ожирением и НМЦ имеют достоверно более высокие уровни кисспептина, а также ЛГ, АМГ, тестостерона, чем девочки-подростки с аналогичным по степени тяжести ожирением без НМЦ. Уровень кисспептина плазмы крови 53,56 пг/мл может быть использован в качестве нового диагностического критерия для прогнозирования риска НМЦ у девочек с ожирением. Пациентки с ожирением и НМЦ достоверно чаще имеют НТУ и гипертриглицеридемию по сравнению с пациентками с ожирением без НМЦ.</p></abstract><trans-abstract xml:lang="en"><p>Background. Menstrual cycle disorders (MCD) are common among adolescent girls.Objective. To study the level of plasma kisspeptin in adolescent girls with obesity and MCD.Design and methods. The study included 80 adolescent girls with obesity. Group 1 — with MCD, group 2 — without MCD. Inclusion in groups 1 year or more after menarche. Laboratory examination included assessment of carbohydrate and lipid metabolism, hormonal examination.Results. There were no significant differences in SDS BMI between the groups (p = 0.486 and p = 0.459). Significantly higher in group 1: impaired carbohydrate tolerance (NTU), hypertriglyceridemia (p = 0.022), levels of kisspeptin, luteinizing hormone (LH), total testosterone, anti-muller hormone (AMH), free androgen index (ISA) (p = 0.001, p = 0.008, p = 0.026, p = 0.014, p = 0.027, respectively). The estradiol level in group 2 was significantly higher than in group 1 (p = 0.012). According to the results of the ROC analysis, the optimal value of the plasma kisspeptin level was found to be 53.56 pg/ml.Conclusion. Obese and MCD patients have significantly higher levels of kisspeptin, as well as LH, AMH, and testosterone than adolescent girls with similar severity of obesity without MCD. The plasma kisspeptin level is 53.56 pg/ml. It can be used as a new diagnostic criterion for predicting the risk of MCD in obese girls.
