Новая коронавирусная инфекция демонстрирует в ряде случаев нестандартные клинические проявления, неотъемлемой характеристикой которых остается наклонность к тромботическим и геморрагическим осложнениям.</p><p>Цель исследования - изучение предрасполагающих факторов развития тромботических и геморрагических осложнений у больных COVID-19.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В основу работы легли наблюдения за 403 больными, находившихся в стационаре по поводу пневмонии вирусной этиологии (COVID-19). Проспективно проанализированы особенности течения заболевания, нюансы коморбидной патологии, маркеры воспаления и коагуляции в основной группе (75 человек), в которой регистрировались тромботические и геморрагические осложнения в сравнении с контролем (328 пациентов), где такого рода последствий не выявлено.</p></sec><sec><title>Результаты</title><p>Результаты. Не было выявлено достоверных различий между группами в отношении возраста и пола. В основной группе регистрировалось большее количество пациентов с избыточной массой тела в сравнении с контролем. Уровни СРБ, ферритина, D-димера у больных, не имеющих осложнений, достоверно реже выходили за пределы референсных значений. Среди осложнений регистрировались спонтанные гематомы, желудочно-кишечные кровотечения, острые нарушения мозгового кровообращения, острый инфаркт миокарда и венозные тромбоэмболические осложнения.</p></sec><sec><title>Выводы</title><p>Выводы: У пациентов с тромботическими и геморрагическими осложнениями COVID-19 увеличивается уровень маркеров воспалительного процесса (СРБ, ферритин). Указанные негативные последствия наиболее характерны для больных с хронической болезни почек, тяжелым течением артериальной гипертензии, ХОБЛ и генерализованного атеросклеротического процесса (перенесенный инфаркт миокарда, инсульт).</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. In some cases, a new coronavirus infection demonstrates non-standard clinical manifestations, an integral characteristic of which is a tendency to thrombotic and hemorrhagic complication.</p></sec><sec><title>Objective</title><p>Objective. To study the predisposing factors for the development of thrombotic and hemorrhagic complications in patients with COVID-19.</p></sec><sec><title>Design and methods</title><p>Design and methods. The work was based on observations of 403 patients who were hospitalized for pneumonia of viral etiology (COVID-19). The features of comorbid pathology, markers of inflammation and coagulation, characteristics of the identified complications in the main group (75 people who had thrombotic and hemorrhagic complications) were prospectively collected in comparison with the control (328 patients), where no such consequences were detected.</p></sec><sec><title>Results</title><p>Results. There were no significant differences between the groups in terms of age and gender. In the main group, a large number of overweight patients were registered in comparison with the control. The levels of CRP, ferritin, and D-dimer in patients with complications significantly exceeded the reference values more often. Among the complications, spontaneous hematomas, gastrointestinal bleeding, acute cerebral circulatory disorders, acute myocardial infarction and venous thromboembolic complications were recorded.</p></sec><sec><title>Conclusion</title><p>Conclusion. In patients with thrombotic and hemorrhagic complications of COVID-19, the level of markers of the inflammatory process (CRP, ferritin) increases.
