Мы предположили, что помимо хорошо изученных форм транстиретинового (ATTR) и AL-амилоидоза, у ряда пациентов может развиваться амилоидное поражение миокарда из ранее не описанных амилоидных предшественников, в связи с нарушением фолдинга структурных белков миокарда. Мы приводим описание клинического случая пациента с гипертрофическим и рестриктивным фенотипом кардиомиопатии, клиникой бивентрикулярной сердечной недостаточности, потребовавшей решения вопроса о трансплантации сердца, у которого были исключены известные формы амилоидоза.</p><p>По данным генетического тестирования у пациента была выявлена протяженная делеция в гене гигантского протеина тайтина (TTN). С помощью методов биоинформатического анализа и молекулярного моделирования мы показываем, как данная мутация потенциально могла привести к разворачиванию соответствующего ему белка и открытию мотивов, склонных к агрегации, для межмолекулярных взаимодействий и, таким образом, придать данному протеину амилоидогенные свойства.</p><p>Полученные результаты позволяют более детально расшифровать молекулярный патогенез ХСН на фоне кардиомиопатий и послужить планированию дальнейших исследований с целью индивидуального профилирования рисков при различных формах амилоидоза и разработки более персонализированного лечения таких пациентов в будущем.</p></abstract><trans-abstract xml:lang="en"><p>Alterations in the proteostasis network and accumulation of misfolded protein aggregates — is one of the new pathogenesis concepts of chronic heart failure. We hypothesis in addition to well-known transthyretin (ATTR) and AL-amyloidosis some patients may represent amyloid lesion in myocardium came from undescribed amyloidogenic precursors due to misfolding of myocardial structural proteins.</p><p>Here, we report on the case of patient with hypertrophic and restrictive phenotype of cardiomyopathy, biventricular heart failure, considered for heart transplant, and excluded known types of amyloidosis. Genetic testing revealed extended deletion in the gene of giant protein titin (TTN).</p><p>We present with the use of bioinformatic analysis and molecular modeling how this mutation could lead to unfolding of corresponding protein and open its amyloidogenic motifs for intermolecular interactions, therefore, provide amyloidogenic ability.
