Острый мезентериальный тромбоз (ОМТ) — состояние, развивающееся при тромбозе в сосудах мезентериального бассейна, что вызывает гипоперфузию кишечной стенки и приводит к вторичным воспалительным изменениям. Смертность при ОМТ составляет 80–85 %. Ранняя диагностика ОМТ затруднена из-за отсутствия специфичных клинических признаков и лабораторных маркеров.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: Выявить предоперационные факторы риска развития неблагоприятных клинических исходов при ОМТ. Материалы и методы: В одноцентровое ретроспективное обсервационное когортное исследование было включено 47 пациентов с диагнозом ОМТ, установленным на основании КТ органов брюшной полости с внутривенным контрастированием или при выполнении диагностической лапароскопии. Были проанализированы следующие данные пациентов: возраст, пол, индекс массы тела (ИМТ), оценка состояния пациентов по шкалам SOFA, APACHE II, EuroSCORE. Проанализированы лабораторные показатели: лактат смешанной венозной крови, маркеры системного воспалительного ответа, уровень D-димера в крови в предоперационном и послеоперационном периодах.</p></sec><sec><title>Результаты</title><p>Результаты: Частота онкологических заболеваний в группе летальных исходов ОМТ была значительно выше, чем в группе выживших пациентов. Перенесенное в анамнезе острое нарушение мозгового кровообращения (ОНМК) достоверно чаще встречалось в группе умерших пациентов. Выявлена взаимосвязь более высоких сывороточных уровней лактата, D-димера, креатинина, лейкоцитоза и прокальцитонина в предоперационном периоде с развитием неблагоприятного клинического исхода при ОМТ.</p></sec><sec><title>Заключение</title><p>Заключение. Основными факторами риска развития неблагоприятного клинического исхода мезентериального тромбоза у пациентов являются: наличие злокачественных новообразований, ОНМК в анамнезе, высокий уровень лактата смешанной венозной крови, креатинина, Д-димеров, прокальцитонина, лейкоцитоз, а также высокие дозы (более 0,5 мкг/кг/мин) норадреналина) в предоперационном периоде.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Acute mesenteric thrombosis (AMT) is a condition that develops with thrombosis in the vessels of the mesenteric pool, which causes hypoperfusion of the intestinal wall and leads to secondary inflammatory changes. Mortality in OMT is 80–85 %. Early diagnosis of OMT is difficult due to the lack of specific clinical signs and laboratory markers.</p></sec><sec><title>Objective</title><p>Objective. To identify preoperative risk factors for adverse clinical outcomes in OMT.</p></sec><sec><title>Design and Methods</title><p>Design and Methods. A single-center retrospective observational cohort study included 47 patients diagnosed with OMT. The following patient data were analyzed: age, gender, body mass index (BMI), assessment of the patients’ condition according to the SOFA, APACHE II, EuroSCORE scales. Various laboratory parameters in the preoperative and postoperative periods were analyzed.</p></sec><sec><title>Results</title><p>Results. The incidence of oncological diseases and history of acute cerebrovascular accident (ACV) were significantly higher in the group of lethal outcomes of OMT than in the group of surviving patients. The correlation of higher serum levels of lactate, D-dimer, creatinine, leukocytosis and procalcitonin in the preoperative period with the development of an unfavorable clinical outcome was revealed.</p></sec><sec><title>Conclusion</title><p>Conclusion.
