Артериовенозные мальформации головного мозга являются редкой сосудистой патологией. Растет число исследований, посвященных особенностям венозного дренирования артериовенозных мальформаций. Многие авторы приходят к выводу о том, что ангиоархитектурные и гемодинамические особенности венозного компонента артериовенозных мальформаций могут влиять на клиническое течение заболевания и тактику лечения таких больных.</p></sec><sec><title>Цель</title><p>Цель. Оценить взаимосвязь особенностей венозного дренирования артериовенозных мальформаций головного мозга с типами клинического течения.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проанализированы результаты обследования 61 взрослого пациента с артериовенозными мальформациями головного мозга супратенториальной локализации, проходившего лечение в нейрохирургическом отделении № 3 РНХИ им. проф. А. Л. Поленова в период с 2014 по 2021 годы. Оценивались демографические и морфометрические характеристики, а также особенности венозного дренирования артериовенозных мальформаций. Пациенты были разделены на 3 группы по типу течения заболевания.</p></sec><sec><title>Результаты</title><p>Результаты. Выявлена взаимосвязь геморрагического типа течения с типом дренирования исключительно в систему глубоких вен (р = 0,034), а также наличием сочетания макрофистулы с венозными лакунами (OR = 4,9; 95 % CI 1,4–17,5, p &lt; 0,05). Эпилептический тип течения был более характерен для пациентов с поверхностной локализацией артериовенозной мальформации (OR = 6,7; 95 % CI 1,4–32,9, p = 0,01). Торпидный тип течения не имел достоверных отличительных особенностей.</p></sec><sec><title>Заключение</title><p>Заключение. Дренирование исключительно в систему глубоких вен и сочетание макрофистулы и венозных лакун является характерным для геморрагического типа течения. Для эпилептического типа течения была характерна поверхностная локализация мальформации. Другие особенности венозных дренажей в равной мере характерны для всех типов течения АВМ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Arteriovenous malformations of the brain are a rare vascular pathology. Many studies focus on the features of venous drainage of arteriovenous malformations. The angioarchitectural and hemodynamic features of the venous component of arteriovenous malformations can influence on the patient management.</p></sec><sec><title>Objective</title><p>Objective. To evaluate the relationship between the features of venous drainage of arteriovenous malformations of the brain and the types of clinical course. Design and methods. Survey included 61 adult patients of the neurosurgical department № 3 of the Polenov RNSI with arteriovenous malformations of the brain of supratentorial localization. Demographic and morphometric characteristics, as well as features of venous drainage of arteriovenous malformations, were assessed. Groups of patients were distinguished according to the type of the course of the disease.</p></sec><sec><title>Results</title><p>Results. The dependence of the hemorrhagic type of course on such factors as drainage exclusively into the deep vein system, as well as the presence of a combination of macrofistula with venous lacunae. The epileptic type was more typical for patients with superficial localization of the arteriovenous malformation. The torpid type of course had no distinctive features.</p></sec><sec><title>Conclusion</title><p>Conclusion. Drainage exclusively into the deep vein system and the presence of a combination of macrofistula and venous lacunae is characteristic of the hemorrhagic type of course.
