Развитие персонализированной медицины и исследование генетических основ сердечно-сосудистых заболеваний являются перспективными направлениями в современной кардиологии.</p></sec><sec><title>Цель</title><p>Цель. Оценить влияние полиморфизма Т786С гена NOS3 на прогноз у пациентов, подвергшихся аортокоронарному шунтированию (АКШ).</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование включено 60 пациентов со стабильной ишемической болезнью сердца (ИБС) и многососудистым поражением коронарного русла по шкале SYNTAX I &gt; 23 баллов. Первая группа: 39 человек — носители генотипов 786СС и 786ТС, вторая — 21 человек с генотипом 786ТТ гена NOS3. У пациентов оценивались выраженность системной воспалительной реакции (СВР) в послеоперационном периоде, динамика изменения фракции выброса (ФВ) левого желудочка, а также клинические данные в течение 12 месяцев наблюдения после АКШ.</p></sec><sec><title>Результаты</title><p>Результаты. Наличие гетерозиготного и гомозиготного вариантов (ТС/СС) гена NOS3 ассоциировано с более выраженной и пролонгированной СВР в послеоперационном периоде: лейкоцитоз в I группе 10,8 ± 0,6 против 8,7 ± 0,6 во второй (р = 0,022) и С-реактивный белок (СРБ) в I выборке 105,4 ± 16,4 против 38,0 ± 1,0 во II (р = 0,001). Шансы развития симптомов острой декомпенсации хронической сердечной недостаточности (ХСН) в течение 12 месяцев наблюдения после выписки были достоверно выше в группе носителей генотипов ТС/СС — в 4 раза по сравнению с носителями генотипа ТТ (ОШ 4,17; 95 % ДИ: 1,05–16,57; p = 0,044).</p></sec><sec><title>Заключение</title><p>Заключение. У пациентов с ИБС и носительством генотипов 786ТC/786СC гена NOS3, подвергающихся АКШ, отмечается более выраженная и пролонгированная СВР в послеоперационном периоде, а также повышенный риск острой декомпенсации ХСН в течение 12 месяцев после АКШ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Development of personalized medicine and study of the genetic basis of cardiovascular diseases are promising areas in modern cardiology.</p></sec><sec><title>Objective</title><p>Objective. To evaluate effect of NOS3 gene polymorphism on the prognosis in patients undergoing coronary artery bypass grafting (CABG).</p></sec><sec><title>Design and methods</title><p>Design and methods. The study included 60 patients with stable coronary heart disease (CAD) and multivessel coronary disease according to the SYNTAX I score &gt; 23. The first group included 39 patients — carriers of the 786CC and 786TC genotypes, the second — 21 patients with the 786TT genotype of the NOS3 gene. We accessed the severity of the systemic inflammatory response (SIR) in the postoperative period, the dynamics of changes in the ejection fraction (EF) of the left ventricle, as well as clinical data during 12 months after CABG.</p></sec><sec><title>Results</title><p>Results. The presence of heterozygous and homozygous variants (TC/CC) of the NOS3 gene is associated with a more pronounced and prolonged SIR in the postoperative period. The chances of developing symptoms of acute decompensation of chronic heart failure (CHF) within 12 months after CABG were significantly 4 higher in the group of carriers of TC/CC genotypes.</p></sec><sec><title>Conclusions</title><p>Conclusions.
