Эндоваскулярная окклюзия ушка левого предсердия (УЛП) с помощью устройства Watchman может рассматриваться как адекватная альтернатива приему оральных антикоагулянтов (ОАК) у пациентов высокого риска.Цель. Описать опыт эндоваскулярной окклюзии ушка левого предсердия системой Watchman в условиях городского стационара, с точки зрения осуществимости и безопасности, а также оценить краткосрочную эффективность данной процедуры.Материалы и методы. В исследование включены пациенты, которым проводилась имплантация окклюзионного устройства Watchman с января 2016 года по ноябрь 2020 года. Эффективность определялась как способность имплантировать Watchman без сохранения остаточного тока. Безопасность оценивалась по частоте возникновения внутрибольничных и 45-дневных событий, включая инсульт, перикардиальный выпот, большие кровотечения и миграцию устройства.Результаты. В исследование включено 22 пациента, средний возраст — 64 года; 10 мужчин и 12 женщин; которым выполнена имплантация окклюзирующего устройства Watchman в УЛП. Показанием к имплантации являлось острое либо рецидивирующее желудочно-кишечное кровотечение в анамнезе у 15 пациентов, кровоизлияние в мозг у 3 пациентов и низкий комплаенс к терапии у 4 пациентов. Успех процедуры составил 95 % [ДИ]: 77,3–100,0; эффективность составила 91 % [ДИ]: 71,0–98,7. По данным контрольной чреспищеводной эхокардиографии (ЧП ЭХО-КГ), через 45 дней УЛП было выключено из кровотока у 21 из 22 пациентов.Заключение. Несмотря на более высокую частоту осложнений при окклюзии ушка ЛП, общий результат в отношении смертности от любых причин у пациентов с имплантированным окклюдером был ниже.</p></abstract><trans-abstract xml:lang="en"><p>Background. Endovascular occlusion of the left atrial appendage (LAA) using the Watchman device may be considered an adequate alternative to oral anticoagulant (OAC) administration in high-risk patients.Objective. To describe our experience with Watchman endovascular occlusion of the left atrial appendage in a city hospital setting in terms of feasibility and safety, as well as assess the short-term efficacy of this procedure.Design and methods. The study included patients who underwent implantation of the Watchman occlusion device from January 2016 to November 2020. The effectiveness of the procedure was defined as the ability to implant the Watchman without maintaining residual current, safety was defined as the occurrence of nosocomial and 45-day events including stroke, pericardial effusion, major bleeding, and device migration.Results. The study included 22 patients: 10 men and 12 women, who underwent implantation of the Watchman occluding device in the LAA. The indication for implantation was a history of gastrointestinal bleeding in 15 patients, cerebral hemorrhage in 3 patients and low compliance to therapy in 4 patients. The success rate of the procedure was 95.7 %. According to control transesophageal echocardiography, after 45 days LAA was excluded from the bloodstream in 21 of 22 patients.Conclusion.
