Определение дилатации левого предсердия (ЛП) эхокардиографическим методом (ЭхоКГ) является одним из критериев диагностики сердечной недостаточности с сохраненной фракцией выброса.</p><p>Цель исследования — определение возможных причин занижения площади ЛП по ЭхоКГ у пациентов с гипертонической болезнью.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В проспективном исследовании у 89 амбулаторных пациентов (57 пациентов с длительность зубца P (з. P) &gt; 0,1 мм и 32 пациента с з. Р ≤ 0,1 мм во II стандартном отведении ЭКГ) проведена эхокардиография с определением площади левого предсердия в 4- и 2-камерных позициях из апикального доступа с оптимизацией выведения ЛП и без нее, а также в 4-камерной позиции из субкостального доступа.</p></sec><sec><title>Результаты</title><p>Результаты. Выявлена сильная положительная корреляционная связь между продолжительностью з. Р по ЭКГ и площадью ЛП по эхокардиографии (r = 0,74). У пациентов с длительностью з. Р более 0,1 мм выявлены различия в площади ЛП в 4-камерной позиции из апикального доступа с оптимизацией выведения ЛП и без нее (22,33 ± 0,68 и 19,02 ± 0,56 см2 соответственно). Максимальная площадь ЛП выявлена в 4-камерной позиции из субкостального доступа (24,41 ± 0,78 см2).</p></sec><sec><title>Заключение</title><p>Заключение. Таким образом, использование соответствия площади/объема ЛП и продолжительности з. Р во II стандартном отведении по ЭКГ, оптимизация выведения ЛП, дополнительное измерение размеров ЛП из субкостального доступа позволят избежать врачам ультразвуковой диагностики занижения размеров ЛП.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. The correct echocardiographic (Echo) determination of left atrial (LA) dilatation is a criterion for diagnosing heart failure with the preserved ejection fraction.</p></sec><sec><title>Objective</title><p>Objective. The aim is to detect probable reasons for underestimating the LA area by Echo in hypertension patients.</p></sec><sec><title>Design and methods</title><p>Design and methods. In the prospective study, the LA area was measured echocardiographically in 89 ambulatory patients (P-wave duration in standard lead II was &gt; 0.1 mm in 57 patients and ≤ 0.1 mm in 32 patients) from the apical four- and two-chamber views with and without focusing on the LA, and from the subcostal four-chamber view.</p></sec><sec><title>Results</title><p>Results. A strong positive correlation was detected between ECG P-wave duration and the Echo LA area (r = 0.74). The LA areas were revealed to be different in patients with P-wave duration over 0.1 mm from the apical fourchamber view with optimization of displaying the LA (22.33 ± 0.68 and 19.02 ± 0.56 cm2, respectively). The maximum LA area was detected from the subcostal fourchamber view (24.41 ± 0.78).</p></sec><sec><title>Conclusion</title><p>Conclusion.
