В статье приведен клинический случай робот-ассистированной лапароскопической радикальной простатэктомии у пациента с предстательной железой объемом 81 см3, со средней долей объемом 39 см3; изложена техника операции, приведены основные периоперационные показатели и кратко изложен период реабилитации.</p><p>Целью исследования было выявить возможные трудности, которые возникают в данной ситуации, поделиться своим опытом и привлечь интерес коллег к этой теме.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Пациент, 73 года, как известно из анамнеза: уровень простатического специфического анигена — 18 нг/мл; по данным трансректальной мультифокальной биопсии предстательной железы, выполненной в марте 2017 года, — ацинарная микрокарцинома предстательной железы (латерально-медиальная зона слева), сумма баллов по Глисону 6 (3 + 3); по данным магнитно-резонансной томографии органов малого таза: предстательная железа 81 см3, ассиметричная с преобладанием левой доли; средняя доля объемом 39 см3 вдается в просвет мочевого пузыря на 4,6 см; простатический отдел уретры смещен влево, не расширен, данных за экстракапсулярную экстензию нет, лимфатические узлы — без особенностей, при остеосцинтиграфии признаков специфического поражения костной ткани не выявлено. Больному была выполнена нервосберегающая радикальная робот-ассистированная лапароскопическая простатовезукулэктомия.</p></sec><sec><title>Результаты</title><p>Результаты. Длительность операции составила 200 минут. Консольное время — 155 минут. Объем кровопотери не превысил 120 мл. Уровень гемоглобина снизился на 7,6 % от исходного значения. Нахождение в отделении реанимации и интенсивной терапии — 1 сутки. Дренажная трубка из малого таза удалена на 2 сутки. Уретральный катетер удален на 7 сутки. Пациент выписан из стационара на 8 сутки после операции.</p></sec><sec><title>Выводы</title><p>Выводы. При проведении робот-ассистированной радикальной простатэктомии у пациентов с выраженной средней долей хирург может столкнуться с несколькими проблемами: большими размерами предстательной железы, трудностью проведения реконструкции шейки мочевого пузыря, особого внимания требует мобилизация устьев мочеточников.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The case of a robot-assisted laparoscopic radical prostatectomy in a patient with an 81-gram prostate with a large medial lobe (39-gram) is presented. We describe the technique of operation, show the main perioperative indications and summarize the rehabilitation period.</p><p>The aim of the study is to detect possible technical difficulties of this operation, to attract the interest of urologist to this topic and to share our experience.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. А 73-year-old male presented to the urology clinic because of elevated PSA level (18 ng/ml); transrectal ultrasound-guided biopsy of the prostate revealed a Gleason score 6 (3 + 3) acinar adenocarcinoma involving left lateral and medial zone; according to the magnetic resonance scan prostate size is 81 g, asymmetric with a prevalence of the left lobe, median lobe size is about 39 g, median lobe prolapses into the bladder for 4,6 cm, the prostatic part of the urethra is shifted to the left, there is no data for extracapsular extension, the lymph nodes haven’t got any changes, there are no signs of specific bone damage according to the bone scan. The patient underwent robot-assisted laparoscopic radical prostatectomy with bilateral nerve sparing.</p></sec><sec><title>Results</title><p>Results. The procedure length was about 200 min; console time — 155 min. Blood loss was 120 ml. The hemoglobin level decreased by 7.6 % of the initial value. Pelvic drain removed on 2rd day. The urethral catheter was removed on 7th day. On the 8th day patient was discharged from hospital.</p></sec><sec><title>Conclusions</title><p>Conclusions.
