Увеличение числа молодых пациенток, излечившихся от основного онкологического заболевания, является главным стимулом для развития и совершенствования современных технологий сохранения фертильности. Одним из таких методов является ортотопическая аутотрансплантация криоконсервированной овариальной ткани. Суть метода заключается в трансплантации предварительно удаленных, замороженных, а затем размороженных образцов коркового вещества овариальной ткани на поверхность яичника после окончания противоопухолевого лечения. Медицинским работникам следует не только помнить о высоких рисках развития бесплодия у пациенток, нуждающихся в лучевой и химиотерапии, но и уметь грамотно и своевременно консультировать этих пациенток. Онкологи должны предлагать данным больным оптимальные методы сохранения фертильности и направлять их к соответствующим специалистам для реализации в будущем ими своей репродуктивной функции. В данном контексте следует отметить, что в настоящее время не существует стандартизованных методик медленного замораживания и витрификации, культивирования in vitro и оценки жизнеспособности овариальной ткани. Каждая лаборатория работает по своим протоколам, что приводит в итоге к тому, что зачастую бывает крайне затруднительным сравнение результатов различных экспериментальных лабораторий, работающих в данном направлении. Метод аутотрансплантации криоконсервированной овариальной ткани нуждается в окончательной разработке и оптимизации для последующего его внедрения в работу онкологических, гинекологических и репродуктологических клиник нашей страны.</p></abstract><trans-abstract xml:lang="en"><p>Treatment of lymphomas has considerably improved survival rates for young women. As a result women’s reproductive function will be damaged due to the gonadotoxicity of chemo- and radiotherapy regimens. Fertility preservation using cryopreservation of ovarian tissue is appropriate for women receiving chemotherapy and radiotherapy for treatment of lymphomas. Progress in assisted reproduction techniques and ongoing rapidly developing research protocols during last years have made possible to offer not only established but also experimental fertility preservation options to young women with Hodgkin’s and Non-Hodgkin’s lymphomas facing gonadal failure. Before patients will be submitted to oncological treatment, the right ovarian cortex is extracted by robotic surgery and then cryopreserved according to standard procedure. As soon as the patient is free of disease, the right ovarian cortex will be thawed and implanted onto the left ovarian medulla using robotic surgery. Ovarian tissue autotransplantation is feasible option to preserve ovarian function and further fertility in young women at risk of developing premature ovarian failure due to chemotherapy and/or radiotherapy.
