При помощи радионуклидной равновесной томовентрикулографии (РТВГ) оценить состояние правых отделов сердца у пациентов со стенозом митрального клапана (МК) ревматической этиологии. Материал и методы. Обследовано 20 пациентов (средний возраст 54,2 ± 8,22 года) с ревматическим поражением МК; 13 пациентов были обследованы до и поле коррекции митрального порока. Все пациенты, до и после коррекции порока, были обследованы методом РТВГ. Результаты. У всех пациентов с пороками МК сердца фиксировали снижение региональной сократимости стенок правого желудочка (ПЖ). Больные с пороками МК по отношению к пациентам группы сравнения имели статистически значимо меньшие значения фракции выброса (ФВ), максимальной скорости изгнания (МСИ) и среднего времени наполнения за 1/3 диастолы (СВН/3), ударного объема (УО) и большие значения конечно-систолического объема (КСО) ПЖ. Коррекция порока реализовалась статистически значимым уменьшением КСО, увеличением МСИ и ФВ ПЖ. Размеры правого предсердия, измеренные по данным РТВГ, достоверно превышали таковые в группе сравнения, а после коррекции порока достоверно уменьшались. Заключение. Метод РТВГ может быть использован для неинвазивной оценки функциональное состояние правых отделов сердца у пациентов со стенозом МК ревматической этиологии.</p></abstract><trans-abstract xml:lang="en"><p>Purpose. To assess the value of gated blood pool SPECT in assessment of right ventricle (RV) contractility in patients with rheumatic mitral stenosis (MS). Materials and methods. The study included 20 patients (mean age 54,2 ± 8,22 years) with rheumatic MS, 13 patients were examined before and after correction of mitral defect. All patients underwent gated blood pool SPECT before and after mitral valve replacement. Results. All patients with mitral stenosis show fixed RV regional wall contractility reduction. Patients with MS, in comparison with control group patients show significantly lower values of EF, the MSI and the MFR/3, SV, and larger values of RV EDV. Mitral valve replacement realized in statistically significant decrease of ESV, increasing of MSI and RV EF. The size of the right atrium, measured by blood pool SPECT, was significantly increased in patients with MS, and significantly decreased after mitral valve replacement. Conclusion.
