При аритмогенной дисплазии правого желудочка (АДПЖ) нарушения реполяризации предшествуют структурным изменениям в сердце.</p><p>Цель работы — оценить изменения желудочковой реполяризации у пациентов с АДПЖ при разной степени выраженности электрокардиографических (ЭКГ) изменений.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Анализировали стандартные ЭКГ и векторкардиограммы (ВКГ) пациентов НМИЦ им. В. А. Алмазова (n=22, 13 мужчин 51±16 лет и 9 женщин 50±12 лет) с установленным диагнозом АДПЖ. Выраженность ЭКГ изменений оценивали по количеству грудных отведений с отрицательной Т-волной, глобальную дисперсию реполяризации — на основе максимального интервала между пиком и окончанием Т-волны, пространственную последовательность реполяризации — по направлению Т-вектора, длительность потенциалов действия (ДПД) приближенно определяли по величине интервалов Q-Tpeak и Q-Tend.</p></sec><sec><title>Результаты</title><p>Результаты. При отсутствии инверсии Т-волны в грудных отведениях наиболее поздняя реполяризация имела место в задних областях основания желудочков, при инверсии Т-волны только в правых грудных отведениях — в латеральной части правого желудочка, при инверсии Т-волны во всех грудных отведениях — в передне-верхушечных отделах желудочков. В зависимости от выраженности ЭКГ изменений длительность реполяризации увеличивалась, причем более короткие изначально ДПД увеличивались в большей степени, чем более длинные; глобальная дисперсия реполяризации при этом не возрастала.</p></sec><sec><title>Заключение</title><p>Заключение. Нарастание ЭКГ изменений реполяризации при АДПЖ — результат неравномерного удлинения ДПД и существенного изменения (вплоть до инверсии) последовательности реполяризации желудочков, не сопровождающегося увеличением глобальной дисперсии реполяризации.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. In arrhythmogenic right ventricular dysplasia (ARVD), repolarization disorders precede structural heart changes.</p></sec><sec><title>Objective</title><p>Objective. The aim of this work was to evaluate repolarization changes in ARVD patients with various degree of electrocardiographic (ECG) changes.</p></sec><sec><title>Design and methods</title><p>Design and methods. The standard ECG and vectorcardiograms (VCG) of patients with a confirmed ARVD diagnosis (N = 22, 13 men 51 ± 16 years and 9 women 50 ± 12 years), observed in the Medical Centre of V.A. Almazov, were analyzed. The degree of ECG changes was assessed by the number of precordial leads with negative T-wave, the global dispersion of repolarization — on the basis of the maximum interval between the peak and the end of T-wave, the spatial repolarization sequence — by T-vector direction, the action potential duration (APD) — on the basis of Q-Tpeak and Q-Tend intervals.</p></sec><sec><title>Results</title><p>Results. At no T-wave inversion in precordial leads, the latest repolarization took place in the posterior regions of ventricular base, at T-wave inversion only in the right precordial leads — in the lateral part of the right ventricle, at T-wave inversion in all precordial leads — in the antero-apical parts of the ventricles. Depending on the degree of ECG changes, the duration of repolarization increased in such a way that the shorter APD increased more than the longer ones; the global dispersion of repolarization was not increased.</p></sec><sec><title>Conclusion</title><p>Conclusion.
