В этиологической структуре инфекционных эндокардитов Coxiella burnetii, возбудитель Ку-лихорадки, занимает важное место, несмотря на ведущую роль стрептококковой этиологии в целом. Тем не менее лабораторные методы не всегда являются достаточно информативными при диагностике коксиеллезного эндокардита, так как рост коксиеллы в гемокультуре затруднен, а серологический ответ при хронической Ку-лихорадке может характеризоваться парадоксальностью. Таким образом, ряд трудностей диагностики инфекционных эндокардитов, вызванных инфицированием C. burnetii, диктует необходимость рассмотрения их эпидемиологических особенностей, особенностей патогенеза Ку-лихорадки. И, хотя для правильной и своевременной диагностики инфекционного эндокардита используют модифицированные клинические критерии Дюка, в последние годы данная диагностическая стратегия дополнилась необходимостью молекулярной детекции C. burnetii, что на сегодняшний момент достаточно широко рекомендовано для введения в практику.</p></abstract><trans-abstract xml:lang="en"><p>Culture-negative endocarditis remains a diagnostic and therapeutic challenge despite recent medical advances. Coxiella burnetii, the causative pathogen of Q-fever, weight with etiological structure of infective endocarditis. However, laboratory diagnosis is often underinformative, because the growth of C. burnetii in hemoculture is difficult and serological response can be paradoxical. The difficulties of diagnostic at Q-fever endocarditis motivate the consideration of the epidemiological characteristics and pathogenesis of Q-fever.
