В рамках такой стратегии первичная профилактика сердечно-сосудистых заболеваний направлена на оценку риска с последующим выбором стратегии превентивных мер и лечения. Существующие алгоритмы оценки с помощью традиционных факторов риска приводят к формированию многочисленной группы лиц промежуточного риска, которая может включать в себя долю лиц высокого риска, требующих более агрессивной терапевтической тактики. Одним из важнейших дополнительных методов обследования, помогающих в стратификации риска, является оценка субклинического поражения артерий, который также принято называть визуализирующими биомаркерами риска. Обсуждаемые в обзоре методы диагностики органных поражений имеют свои достоинства и недостатки, разную доказательную базу и экономическую эффективность, что приводит к отсутствию единых алгоритмов и рекомендаций. Наличие большого количества обсуждаемых маркеров обусловлено, с одной стороны, востребованностью разработки критериев для активной первичной профилактики, с другой стороны - необходимостью учета экономических затрат при скрининге на уровне популяции. Продолжается поиск визуализирующих биомаркеров, которые, с учетом оптимального соотношения чувствительности, специфичности и стоимости, смогут быть использованы для скрининга на популяционном уровне.</p></abstract><trans-abstract xml:lang="en"><p>High risk strategy at present is one of the most effective strategies of prevention. Primary prevention of CVD (coronary vascular disease) consists of evaluation of risk and following choice of preventive measures and treatment of individuals. Current risk scores by using traditional risk factors, such as SCORE etc., lead to forming of numerous groups of intermediate risk patients which contains underestimated high risk patients, who need more aggressive therapeutic measures. Subclinical organ damage detection which is also called visualized risk biomarkers is one of the most important additional methods of examination for risk stratification. Every discussed method of organ damage detection has his own advantages and disadvantages, different base of evidence and economic effectiveness, which leads to absence of universal guidelines. Existence of great number of markers is determined by some reasons: first and foremost, by necessity of development of effective primary prevention criteria; secondly, by necessity of economic expenses accounting on population screening level.
