В настоящее время отмечается рост числа зарегистрированных случаев гипертрофической кардиомиопатии, вероятно, обусловленный совершенствованием диагностической техники, методик обследования, а также увеличением общего количества пациентов. Золотым стандартом хирургического лечения остается открытая трансаортальная миоэктомия, которая имеет ряд недостатков в виде ограниченной визуализации, в ряде случаев приводящей к невозможности выполнения радикальной резекции гипертрофированных мышц. Необходимость расширения объема резекции, улучшения качества визуализации, снижения частоты рецидивов заболеваний определяет актуальность разработки техники нового доступа к межжелудочковой перегородке.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Изучить возможности нового расширенного доступа к межжелудочковой перегородке с эффективным восстановлением аортального и митрального клапанов при хирургическом лечении гипертрофической кардиомиопатии в эксперименте.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. С марта 2017 г. по май 2023 г. на базе Центра доклинических и трансляционных исследований ФГБУ «НМИЦ им. В. А. Алмазова» Минздрава России было выполнено две серии экспериментов с целью исследования расширенного доступа к межжелудочковой перегородке с восстановлением аортального и митрального клапанов.</p></sec><sec><title>Результаты</title><p>Результаты. Разработанный расширенный доступ к межжелудочковой перегородке продемонстрировал свою эффективность на экспериментальных анатомических моделях. Подтверждена возможность эффективного восстановления комиссуры аортального клапана и передней створки митрального клапана после выполнения расширенного доступа без значимого влияния на планиметрию фиброзных колец и функцию данных клапанов.</p></sec><sec><title>Заключение</title><p>Заключение. Разработанный расширенный доступ позволяет улучшить визуализацию межжелудочковой перегородки у пациентов с узким фиброзным кольцом аортального клапана, средними и дистальными формами гипертрофической кардиомиопатии. Восстановление клапанного аппарата (митрального и аортального клапанов) при помощи нового доступа является возможным и эффективным.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Currently, there has been an increase in the number of reported cases of hypertrophic cardiomyopathy, probably due to improvements in diagnostic techniques, examination methods, and an increase in the total number of patients. The gold standard for surgical treatment remains open transaortic myectomy, which has a number of disadvantages, such as limited visualization, in some cases leading to the inability to perform radical resection of hypertrophied muscles. The need to expand the scope of resection, improve the quality of visualization, and reduce the frequency of disease recurrence determines the relevance of developing a new technique for accessing the interventricular septum.</p></sec><sec><title>Objective</title><p>Objective. To investigate the possibilities of new extended access to the interventricular septum with effective repair of the aortic and mitral valves in the surgical treatment of hypertrophic cardiomyopathy in an experimental setting.</p></sec><sec><title>Design and methods</title><p>Design and methods. From March 2017 to May 2023, two series of experiments have been conducted at the Center for Preclinical and Translational Research of the Almazov National Medical Research Centre to investigate extended access to the interventricular septum with repair of the aortic and mitral valves.</p></sec><sec><title>Results</title><p>Results. The developed extended access to the interventricular septum demonstrated its effectiveness on experimental anatomic models. The possibility of effective repair of the aortic valve commissure and the anterior mitral valve leaflet after the extended access without a significant impact on the geometry of the fibrous annuli and the function of these valves has been proven.</p></sec><sec><title>Conclusion</title><p>Conclusion. The developed extended access allows for improved visualization of the interventricular septum in patients with a narrow fibrous ring of the aortic valve, mid-ventricular and apical forms of hypertrophic cardiomyopathy. Restoration of the valve apparatus (mitral and aortic valves) using the new access is possible and effective. Based on the results of this study it seems promising to conduct preclinical studies (in vivo on animals).
